The BonaVita Membership Program is designed to help you save on your favorite treatments, services and skin care while budgeting toward your ongoing aesthetic care.
For $100/month, your preferred credit card will be automatically charged on the 1st of each month, and the funds will be added to your BonaVita Wallet. These funds can be used toward qualifying in-office services and skincare products.
All new members are subject to a one-time enrollment fee of $50.00 due at the time of enrollment. In addition, an annual membership fee of $50.00 will automatically recur each year during the client’s original enrollment month. The annual membership fee covers administrative and transaction-related costs for the calendar year and is non-refundable. This fee may not be applied toward any services, treatments, or product purchases.
BonaVita Members receive the following benefits:
• Dysport, $7.00 per Unit (year-round discount)

BonaVita Beauty Skincare Products 10% Off (year-round discount)
• One MICC Vitamin Shot (one per season)
• One Full Syringe of Filler, $50 Off (one discount per season)
• Facial Service, 10% Off (one discount per season)
• IV Infusions, 10% Off (one discount per season)
• Laser Hair Removal, 10% Off (one discount per season)
• Microneedling, 10% Off (one discount per season)
Program Details:
• No cancellation fees.
• Funds roll over each month and never expire while your membership is active.
• If your treatment exceeds your wallet balance, you simply pay the difference.
• If your treatment is less than the wallet balance, the remaining funds stay in your wallet for future use.
• Seasonal discounts are valid during the following months: February, May, August, and November. These perks must be used within the specified seasonal window and do not roll over.
• Funds are non-transferable and cannot be shared with or gifted to another client.
• Membership funds cannot be applied to cancellation or no-show fees.
Billing Process:
• Your credit card will be charged automatically on the 1st of each month.
• Payments must be made using a credit card; we do not accept debit cards or cash for membership billing.
• If payment is declined:
• The system will retry automatically on Day 2.
• Our office will attempt to contact you to update payment information.
• If payment is not resolved by Day 3, your membership will be terminated.
• Any remaining funds in your BonaVita Wallet will be forfeited and cannot be used toward future treatments or purchases.
• Office closures or holidays do not affect billing dates. If the 1st falls on a holiday or closure, the system will still process payment, and any declined charges will follow the same Day 2 and Day 3 protocol.
BonaVita Membership Cancellation Policy After “Cooling- off Period”
• After the three-business-day cancellation window required under Connecticut General Statutes § 21a‑219 has expired, members may cancel their membership at any time.
• A minimum 4-month commitment is required to join the BonaVita Membership Program.
• Early cancellations made before the 4-month minimum may be subject to a charge equal to the difference in savings received.
• After the 4-month minimum is met, you may cancel your membership at any time by calling our office directly at (860) 333-8944. No other form of cancellation (e.g., text, email, social media) will be accepted.
• Once canceled, any remaining balance in your BonaVita Wallet must be used within 90 days. After that time, BonaVita Aesthetics reserves the right to withhold unused funds.
• If cancellation is initiated by direct contact with the Bonavita Aesthetics office via phone, the member will have ninety (90) days from the cancellation date to use any remaining funds in their member account (“Member Bank”). After 90 days, any unused funds will be forfeited.
• If cancellation is initiated solely through the online portal, and the member does not contact Bonavita Aesthetics directly via phone, the member forfeits all rights to any remaining Member Bank balance as of the cancellation date. The 90-day usage window does not apply, and any unused funds are deemed nonrefundable and forfeited at the time of portal-based cancellation.
• To retain your 90-day usage window after canceling, you must notify our office directly. Cancellations made only through the online portal will result in immediate forfeiture of any remaining balance.
Special Circumstances:
In the event of significant life changes (e.g., pregnancy, military deployment), you may request a pause in your membership. Each case will be individually reviewed and approved by management. Membership perks and payments will resume once the pause period has ended.
Additional Terms:
• Prices, discounts, and terms are subject to change without notice.
• No refunds will be issued for unused funds or missed seasonal perks.
